Position Description: The Therapeutic Mentor offers structured, one-to-one, strength-based support services between a to youth (up to the age of 21) for the purpose of addressing communication, daily-living, and social needs in any setting where the youth resides, such as the home (including foster homes and therapeutic foster homes), and in community settings, such as schools, child care centers, respite settings, as well as other culturally and linguistically appropriate community settings.

Goal of the Position: To develop relationships with at-risk youth that will promote positive family and peer relationships, development and decision-making.

Therapeutic Support:

• Empowering them to resolve current issues and to develop coping strategies for the future.
• Provide a model for a healthy, trusting relationship through clear communication and setting appropriate boundaries.
• Provide training, assistance and/or supervision to youth in the areas of daily living, personal care, financial management, community and social integration and/or job skills development.
• Establish a therapeutic relationship with the youth receiving services to ensure effective guidance, direct support and service delivery.
• Engage in activities outside of the home to help expose youth to new opportunities and experiences.

Office Support:

• Develops Individual Service Plan according to initial assessment.
• Provide weekly pre-vocational skills and cognitive skills training.
• Provide support to youth, to ensure objectives are met in accordance with the Service Plan.
• Maintain weekly progress and service notes, monthly report, and any other appropriate documentation in accordance with program policy.

Responsibilities:

• Bi-weekly supervision with the Program Coordinator.
• Maintain confidentiality of participants.
• Clear communication with the family and treatment team.
• Weekly visit with youth.
• Weekly submission of documentation.
• Occasional treatment team meetings.

Qualifications

• Must be 21 years of age
• Bachelor’s Degree preferred. Minimum education; High School Diploma/GED
• Must have one or more years of applicable experience
• Exceptional organizational, written, verbal, and interpersonal communication skills
• Solid relationships with local community organizations a plus
• Must have valid Driver’s License with an acceptable driving record
• Successfully complete a criminal background check
